Our facility was faxed a request for a Physician’s authorization for DME request. Patient was contacted to see if he contacted this company. He stated he has and requested a knee brace. They sent a order for Prefabricated lumbosacral support w/ lateral control. I called the company and the number states it is no longer in service.
