Please call us toll free 1-866-955-5478 to claim your reward to you as soon as possible. Please call 1-866-955-5478 to claim your reward. Your claim number is ___.
On the other side of the card there is no return address. Failure to call will result in automatic forfeiture of this reward. Please call at once for details.
