Company listed on return address is Senior Information Processing. Letter states second notice about "New Medicare Changes That Affect You!" First of all, I’m not even on Medicare or even Medicaid. They want me to fill out a card with my name, date of birth, spouse’s date of birth, phone & address. I would never do this, but am concerned since I’m not even on Medicare or Medicaid.
