Type of a scam Fake Invoice/Supplier Bill

We received a bill for a surgical assistant for a surgery my husband did have. Indicators it was a scam: 1. The business was not listed. 2. The date of service was incorrect and only titled vaguely "date." 3. The address is a home address. 4. The email is a g-mail account. 5. I could not locate their phone number as listed. 6. The claimed they billed our insurance and no such claim was listed with our insurance. 7. We had already hit our max out of pocket, so insurance was covering all medical charges related to this procedure. 8. There were multiple typos. 9. There was no business website or bill-pay website. 10. They offered, with exclamation points and in all caps, a 20% discount if paid in full, with a credit card within 10 days.
