I was contacted by Michael Collins of the PCH, New York office. I was told that I had won 4.5 million in the $7000 Set for Life Promotion, bonus check. My Claim # 0095US
I was told that taxes in the amount of $13,395 has been withdrawn by the IRS, so no further taxes required. I then was told that I would need to pay 1% State Tax in the amount of $1010 & that 99% was paid by PCH.
I asked for something in writing and I was told to go to the UPS store in Appleton and look for an email from PCH.
