Received a letter from Ipsos RDA notifing me I was a $1,000 winner drawn from the DTE Energy appliance survey. I do remember filling out several pgs for this survey. In the letter notifing me was a W 9 form for the IRS to be filled out by me for this company to hold unto , it asked for my name , address and SSN ,( was told it would be secured) but my winnings had to be reported for tax withholding purposes . Contacted DTE Energy to verify this Co. and see if they had notified them of the winners of their survey and as far as they knew no research firm had been contacted to do a survey . The name to contact on the letter was Kevin Wachowiak 1-800-669-7324
