I received a notice in the mail from US Bank, stamped as Important Tax Information, 1099-17. Inside it had the last 4 digits of my social security number and the last 4 digits of a credit card from a completely different bank. This was a 1099 misc with $5 interest withheld. The form included an ein#. I do not have an account with US Bank or any credit cards with them. I contacted the 800 number and the first thing they asked for was my social security number. I would not give it to them. I went to the bank that has my credit card and they could not tell me anything. I ended up going to the closest US Bank, which was in Appleton, WI and was told that I was not in their system. I feel like this is fraud and that they were trying to get my personal information.
