I received a letter claiming that WA State DSHS has been investigated by the US Dept. of Labor as a result of which I am due $490.22 in back wages. They requested I send them a copy of my "soc sec card, picture ID, pay stub, W-2 or other documentation that includes my name and SSN" before they send me the check.
