not much information on company or reason for letter…vague reference to benefits I am entitled to not by government funds ,mentions $35000 in final expenses,mentions ‘state regulated’.

Form to fill out resembles a tax form or government refund form…asking for my husband’s personal information, my information: name, age,phone, has a Qr code with the Number: PL40889B134554HK underneath for some reference at the bottom of form..

Looks suspicious no other information ..no reference to selling or premium cost .. or company information

I got a notice from this company about funeral insurance for $35,000, that stated it is tax free money. It asks for your name address and phone number and is to be returned In a postage free envelope to direct mail processing. It looks like it is a tax form so people probably fill it out and return it. I’m sure it is a scam to get your information. Tear your name Off the form, and return the blank form in their postage paid envelope. That way they pay postage for nothing!

It should be illegal to make a form that looks like a tax form!

I googled the address on "True People Search" website and it showed the address associated to a 78 year old man name "William A. O’Keefe Jr." The following phone number associated with Mr. O’Keefe on the website was"(404) 766-0875."
