Type of a scam Identity Theft
On 10/13/15 I tried to download an application form from the State of Illinois. I was unable to get the form downloaded. I typed in the form number on Google (CMS100) and the form came up in the pdffiller site. They state that the form can be saved, emailed, printed, etc. so I proceeded to fill the form out. When I completed the form I was unable to print or save the form. I was redirected to a page that states I have to subscribe for a year in order to print the form. Now this company has all of my personal information (name, address, s/s number, etc.) I am concerned that my identity will be compromised because I didn’t subscribe. They should have been honest up front about having to subscribe and I would have left the site without filling out the form. Since this just happened this morning I have not lost anything but I am physically sick over this.
