Received a letter in the mail with Portugal postmark, from International Lottery Board in Madrid Spain. Supposedly I won over 5 million Euros. I need to contact the claim agent, Don Pablo Lucas, to claim the prize by July 5. The payment process form asks for my bank info, my birth and "expiration" dates, my contact information, along with my "next of kin". The form is to be faxed with a copy of my ID.
