Received a card by mail which had our business name (which we just opened) and UBI number.They leave toll free number 1-888-803-7922 and key code number and they wrote : Please call immediately to avoid potential fees and penalties and U.B.S.1532.8 . Was send from Spokane but stamp was paid Tampa,FL PERMIT#3311
