Postmarked from South Africia, with a check from TRICARE POB 7889 Madison WI 53707-7889 in the amount of $3,750.00. check to be drawn on JPMORGAN CHASE BANK, NA, COLUMBUS OH. Letter advised her to contact claim agent Ms Alice Dubery @ 1-877-217-5959. and to adhere to her instructions strictly. Attaching photos of letter and check.
